Arrive about 10 minutes prior to your scheduled appointment time if you have not completed your previsit paperwork available at our Quick Check-In link.
Bring comfortable and loose fitting clothing. Shorts for treatment of your knees and ankles. Tank top for shoulder and neck. Gowns are provided for middle and low back evaluation.
You will meet with your physical therapist, occupational therapist, or certified hand therapist for an initial evaluation that includes a review of your medical history and establishment of an individualized plan of care and joint setting of goals based on objective measurements of your range of motion, strength, function, and additional specific tests where warranted.
Your written assessment will be completed and forwarded to your referring physician within 24 hours.
Follow-up visits may be scheduled at the conclusion of your evaluation visit as prescribed by your physician.
Follow-up Visits
Follow-up visits, generally shorter in length than your intial evaluation visit may be conducted with your therapist and / or additional staff including physical therapist assistants, or other staff as necessary.
Your course of therapy includes patient education during which you learn about your diagnosis and its treatment, instruction in an individualized home exercise program, review and progression of your home program, application of hands-on techniques, and use of physical agents to address your symptoms and underlying causes.
Your follow-up visits include periodic reassessment of your progress and revision of your program goals and plan of care. This information will also be forwarded to the referring source.
